

Patricia Ann Collins (née Smith) — lovingly known as Pat to her friends and family, and Mom Mom Patsy to her grandchildren and great-grandchildren — peacefully passed away on August 18, 2026, at the age of 82.
Born in Philadelphia, Pat grew into a woman whose warmth, steadiness, and quiet strength shaped the lives of everyone around her. She devoted herself to her family, friends, and to those in need. She lived her life sharing her love, laughter, and kindness and brightened the lives of everyone she met along the way.
Pat will be deeply missed by her daughters, Tina Faleshock (Brian) and Jennifer Weaver (Brian); her grandchildren Jordan Faleshock, Joshua (Brianna) Faleshock, Rachel Faleshock (Robert McDonough), and Shane (Katelyn) Collins; and her five great-grandchildren: Michael, Caleigh, and Lucas Collins, and Camila and Lincoln Faleshock and friend Anna Marie Collins.
She is also survived by her sisters Dolores Chilelli and Stella Madeline (Rolf) Talarek, and her brothers John Smith and Thomas Smith (Patricia Griebel) and her many nieces, nephews, great nieces and nephews — all of whom shared in a lifetime of family bonds, stories, and love.
She is predeceased by her parents Stella and Harold Smith, her sister Mary, her brother Harold Smith, Jr., Her brother-in-law William Chilelli, sisters-in-law Carol Smith, Lorraine Smith and Janice Smith, her former husband William A. Collins, Jr., her son Michael W. Collins.
Pat’s legacy lives on in the generations she nurtured, the memories she created, and the love she gave so freely. Her family will carry her spirit forward with gratitude for the many ways she shaped their lives.
